Remember that the quality of the poem is dependent on the material that you supply. More good details give us more chances to make the poem full and alive. If you request a longer poem, make sure that you  give us plenty of material.

1. Person’s name for whom the poem is to be written.

2. Nicknames if you want any to be included in the poem.

3. How did you and the person meet? What is your relationship?

4. Give some significant history between you and the person, for example, special events you have shared together, achievements, funny times together.

5. What qualities do you most associate with this person? How have you seen these qualities demonstrated in the person’s life or in your own life?

6. What are some words or phrases that this person may say often enough that when you think of the person you think of them saying these words?

7. What is the best thing about your relationship with the person? Why?

8. What are your wishes for the person? What are some things you want to say to them?

9. Are there any other important things that you wish to be included in the poem?
